CLCH Hertfordshire Adult Community Services -CLCH NHS Trust provides a range of Community Services, and the Hertfordshire Division is recruiting! We are currently looking to expand our fantastic nursing team, on the search for motivated, proactive, and enthusiastic individuals who are keen to develop and grow in the world of community nursing. This post offers experienced nurses the opportunity to develop or expand clinical skills, ranging from Wound care, Palliative care and Iv therapies alongside developing leadership and managerial skills. There is also the opportunity for successful applicants to explore the Specialist Practitioner District nursing qualification in the future if this isnt something they have already achieved. • To provide leadership and management to a team of community nurses • To be responsible for the team’s assessment, planning, implementation, and evaluation of individualised nursing care to patients living in the community. The service is provided seven days per week 8am – 6pm • To be responsible for the team’s maintenance of patient and other records • To participate in the long-term management of patients within the locality • To have continued responsibility for a defined caseload • To be committed to teamwork and demonstrate good communication at all times. Develop networks for effective communication between health, statutory and voluntary services including patient forums.
